  • Step 1: Register: REGISTRATION FORM; when the form is completed correctly, it will show you a "Thank You" page with a link to return to this page.  Then, proceed to step 2.
  • Step 3: Complete: "MTAC REQUIRED FORMS", These forms are "Required" to participate in our activities. "Signed" originals of each form "must" be in our possession prior to the activity in order to participate. Signed originals need to be mailed to the address on Page 1 of the first form.
